2008-2009 Medical Release and Waiver Form
Dear Parents, The beginning of the new church year is here and we have some
incredible things planned for the coming year in the student ministries
department. I would like to meet with you as parents of teens in the church on
Sunday, June 8th, immediately following the morning worship service. You
will receive a packet that will include a student permission slip/medical waiver
and media waiver, along with a basic calendar of events for the 2008-2009 church
year. Another thing you will receive is a summary of our vision and mission as
a student ministry and an outline of the message series that we will be covering
this year. If you have any questions or comments that you would like to be made
part of the meeting, please email me at
